We¹ve found that installing CS4 and then applying both the Acrobat 9.0.1 and
9.1.0 updates has fixed this issue for us.

Earlier voodoo still applies:

    1.) Install CS4
    2.) Install updates
    3.) Launch Photoshop or some other non-Acrobat application first
    4.) Launch Acrobat and Distiller.

I still launch Acrobat and Distiller as an admin the first time but that may
not be necessary.
